The Kissito Post Acute  Model™

Kissito Healthcare has spent the last 12-18 months developing a patient centered post acute model of care. This patient-centered approach emphasizes collaboration, coordination and partnership among patients, families, hospital caregivers and post acute healthcare teams to produce smoother care transitions, improved outcomes and reduced costs. The model contains 5 phases all designed to insure a safe and effective tranisition for the patient back to home.

*Please note the Transition and Home phases of the Return to Home Program are still being studied and have yet to be implemented. However many components are in place and are currently being used to deliver patients back to their homes at the highest possible level of function.
